Alumni Award Nomination Form

 DEADLINE FOR SUBMISSION: May 10, 2007

This form is for the purpose of submitting nominations for the Alumni of Point Loma (APL) Award and the Distinguished Achievement (DA) Award. The nominee must have recieved a degree from this insitution. The difference between the APL Award and the DA award is that the DA Award is considered a lifetime achievement award. Each nominee must meet the following criteria:

1. Strong Christian testimony
2. Outstanding accomplishments professionally, academically, or in service to a non-profit organization
3. Impact on people's lives

NOTE: The Alumni Association does extensive research on each nominee, but this nomination will be the first thing each Awards committee member will read about your nominee. Also, please keep your nomination secret.
