2) or alternatively, by regular mail, using the form shown below.

Please download this registration form and either print or type in all the information that is requested (even if you have participated previously in POS). Mail the completed form to Dr. Michael McConnell, along with your $75 registration fee (see mailing address below). The $75 registration fee covers all eight events for the year (pre-seminar refreshments, seminars and catered dinners). Checks should be made payable to “PLNU-POS”.
Application Receipt Deadline is September 15, 2008. Please note: Due to logistical constraints, only the first one hundred persons who complete the registration process (including payment of the $75 registration fee) will be guaranteed acceptance into the Program.
