Christian stewardship. Please know that we also take your interest
seriously and desire to give you a quality interview. To gain the
maximum benefit from your interview please take note of the following:
You will be asked to complete a 5-page questionnaire.
PAGE 1
is your personal information. Complete it as far as possible. If you are not clear about any item, leave it blank.
Be prepared with your Social Security number(s), previous marriage
termination date(s) and children’s birth dates (by current or previous
marriage).
NOTE: Most want “identical provisions” in their wills unless special circumstances exist.
PAGE 5 IS IMPORTANT CONFIDENTIAL INFORMATION.
This helps us make suggestions for your greatest benefit.
Be prepared with “Real Estate” information: the date(s) purchased,
original cost (or “basis” estimate). FMV (Fair Market Value today), and
Mortgage Balance(s). Check (x) if it is separate property (his or hers
or in joint tenancy.)
NOTE: This is the only item you enter “purchase date” or “cost.”
Under “Insurance” if possible, enter the company (not policy numbers),
List under FMV (face/pay-out) value, and check (x) whether his or hers.
For “Stocks, Bonds, Mutual Funds and Securities,” list
only those purchased outright (NOT tax deferred pension or IRA funds).
If you have a portfolio, just list it as such, not individual stocks.
Again, FMV= best estimate of today’s value. Check (x) ownership.
List Retirement Accounts (IRA’s and tax deferred pension funds)
separately for each of you. Again, FMV= best estimate of how much is in
the account(s) today. No account numbers are necessary. Check (x)
ownership.
List under FMV the total of your CD’s
separate. List only Savings or Bank Accounts that maintain $1,000 or
more on a regular basis. If you have a Business that has a value if
sold (inventory/building/good name), list an estimated FMV.
Pages 2, 3, and 4 are best completed during the interview and cover:
- Where (to whom) and in what manner you want your assets distributed.
- Where your estate would go in case of a “common tragedy” that left no “issue”.
- Any “charitable” gifts you may desire to make.
- A Successor Executor/Trustee and an alternate with their addresses.
- Nominees for powers of attorney (legal and health) if to different than trustee.
