Welcome to the Athletic Training Clinic forms page. These forms will help to make your transition into the new school year run smoother. A few very important things that you need to know:

1. The following forms need to be printed, filled out, and brought with you to physicals  in the Fall
2. If you choose to get your physical done at home you must use the forms provided for your physical to be valid
3. You must bring an updated copy of your insurance card with you to physicals
4. If you have an HMO and you are not from San Diego, you must make sure you choose a primary care physician in San Diego before the school year starts
5. If you have an HMO from another state besides California then you must transfer your coverage to California for it to be valid
