A passport is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid U.S. passport is required to enter and leave most foreign countries, and only the U.S. Department of State has the authority to grant, issue and verify United States passports.©

- Complete a passport application. All information must be accurate. Do NOT sign the application form until the Passport Acceptance Agent instructs you to do so
- Apply in person at a Passport Application Acceptance Office. The nearest office to PLNU is at the U.S. Post Office on Midway Drive. Other locations can be found at http://iafdb.travel.state.gov/
- You will need an original or certified copy of your birth certificate or a previous passport. If you can't locate your birth certificate, contact the county recorder's office in the city of your birth or access www.vitalcheck.com online and request a certified copy.
- Include two passport-style photos. These can be taken in our office for $8
- Bring your driver's license or state ID
- Passport application and execution fee totals $97. Payment methods: Credit Card, Debit/Check Cards, and Checks (personal, certified, or cashiers' )
