All students, click here to upload copies of your Immunization Records and your Health Insurance Card.

We need both a copy of your immunization records and a copy of your health insurance card in order to complete your records here at Point Loma.  Providing these in a timely manner will ensure you’re right on track to receive your first semester schedule and housing assignment!  

Just scan the item (you can even scan a front and back and submit both if you need to!) and save it on your computer.  From the portal link above, click on that item title, and you’ll be taken to an upload box where you can submit your forms! 

Don’t have a scanner at home?  Try your local library or church!  Many of them have scanners you can use easily.  Still no luck?  All FedEx/Kinkos locations have scanners available too!  STILL can’t find a scanner?  You can also upload a photo of your records.  Just be sure the photos are close ups and clear enough to read all pertinent information!  If none of those options work, you can mail a copy to us at PLNU Wellness Center, 3900 Lomaland Drive, San Diego, CA, 92106.

Since you’ll be spending a lot of time with us on campus, we want to be sure you’re healthy and well. That’s why our on campus Wellness Center is here to serve you. Our missions says it all. The Wellness Center offers a number of discussion groups to support you as you transition to college life and grow, and as various changes or life circumstances may arise. We also offer a number of services to students —physicals, travel medication, mental health services, nutrition counseling, and more!

The Wellness Center is a limited service provider to our Undergraduate students. It is strongly recommended that any student with a chronic physical or psychiatric condition establish a relationship with a local health provider prior to starting school.
