Please fill out the form below (if you are a high school senior during the fall of 2013) to let us know that you are interested in the PLNU Science Honors' Scholarship Weekend for Biology, Chemistry or Environmental Science. If you are interested in Math, Physics or Computer Science - please check their websites for a different form. This form must be submitted by Wednesday, December 18, 2013 in order to be considered for our scholarship opportunity. VERY IMPORTANT: Please make sure that you receive a thank you from us right after you click "submit", otherwise that could mean that we haven't received your information.
