PREPARING FOR GOOD HEALTH AND SUCCESS AT PLNU

Congratulations upon your acceptance to Point Loma Nazarene University!  PLNU places a high priority on the health and safety of our students.  We require enrolled undergraduate students to have physical examination forms, health insurance coverage and up-to-date immunizations on file in the Wellness Center prior to the start of classes. Our medical team is excited to help you maintain high levels of good health and wellness as you pursue your academic goals. Knowing your needs before you arrive on campus will help our team to promote your total health and wellness throughout your journey toward graduation.  In planning for your holistic success, the University requires all undergraduate students to complete and submit the following by July 1st for students entering fall term: 


  • Official PLNU Undergraduate Student Health History Form
  • Immunization and TB Verification Form
  • Required Physical Examination Form (completed within 1 year of entering the university)
  • Front and Back Copy of Health Insurance Provider Card (for students waiving PLNU Student Health Insurance Plan)

 

PHYSICAL EXAMINATION, HEALTH HISTORY, IMMUNIZATION AND TB VERIFICATION

Before you start your journey to graduation at PLNU, make sure that you have completed your Health History, Physical Examination, Immunization and TB Verification and provided proof of Health Insurance  (select your own health insurance plan or ours). 

 

HEALTH HISTORY AND PHYSICAL EXAMINATION

Recent physical examination and accurate health history information provides a baseline for your health and wellness.  PLNU requires incoming students to complete and submit a recent physical examination (completed no more than one year prior to the start of school) and health history information using the official forms on the student portal (my.pointloma.edu).  Both forms require the signature of your health care provider!  Submit Health Information forms to the Wellness Center  no later than JULY 1st for students entering fall semester.  Students not having complete information on file in Wellness by the required date risk significant delays in housing and class registration.


 

REQUIRED IMMUNIZATIONS

  • Tetanus (Tdap must be within the last 10 years)
  • MMR-Measles, Mumps, Rubella (verification of 2 doses for students born after 1956)
  • Current Tuberculosis (TB) screening (within the past year
  • Meningitis (Menactra) within the past 5 year
  • Hepatitis B vaccine - series of 3 doses
  • Varicella vaccine - series of 2 doses (or titer if student had disease)

PLNU requires verification that incoming undergraduate students have received all of the above listed immunizations.  You can access the Official Immunization and TB Verification Form on the student portal (my.pointloma.edu). Take the form to your doctor who will sign off to verify that you have received the immunizations and are ready to begin your college studies at PLNU. Do you have questions about immunizations? PROOF OF HEALTH INSURANCE COVERAGE

All undergraduate students at PLNU are required to have health and injury insurance coverage. For your convenience, a PLNU Student Health Insurance Policy is automatically billed to undergraduate student accounts each semester.  Students must indicate their choice to keep or decline PLNU Student Health Insurance coverage prior to the start of the semester in order to receive a credit on their student account. Choosing to decline the PLNU insurance policy means that you must provide proof of health insurance coverage to the Wellness Center no later than JULY 1st.

 

SUBMITTING HEALTH INFORMATION TO THE WELLNESS CENTER

We make submission of documents easy.  Go to the student portal (my.pointloma.edu) to access a printable copy of the Official Health History, Immunization and TB Verification and Required Physical Exam Form.   Print copies of the forms to take to your doctor’s appointment.  Make sure that all signatures are completed.  Forms that are incomplete or not signed by the appropriate health care provider will not be accepted. 

If you are waiving the PLNU Student Health Insurance Plan, you must include copies of the front and back of your insurance card to the Wellness Center, along with your name and ID#.  Remember, all forms are due July 1ST

RECOMMENDATIONS FOR FRESHMEN STUDENTS PREPARING FOR MEDICAL CARE WHILE AWAY FROM HOME

The PLNU Wellness Center provides limited primary care and brief counseling services for undergraduate students.  Freshmen students with medical/dental/psychological conditions requiring frequent follow up appointments should select a primary care physician in addition to other health care providers a semester in advance of attending the university. For example, students entering in the fall should use the previous spring and summer to connect with local health care providers.  Be advised that parking on campus is unavailable for freshmen students, and that the Freshmen Vehicle Policy will remain in effect.  It is strongly recommended that transportation be arranged in advance through  ZipCar or Car2Gofor physical therapy, medical, counseling, chiropractic, dental and other frequent health related appointments.  Additional transportation suggestions include:
