All LoveWorks participants are required to obtain the proper immunizations for their trip. We will provide the list of required immunizations at the beginning of training in the Spring semester. Below are the proper steps to begin the immunizations process:
STEP 1: Locate your immunization records. You should have some kind of booklet or document where your immunization history from birth is recorded. This might be something your parents/guardians are keeping for you.
STEP 2: Check to see if you are already current with any of the immunizations required for your trip. Required immunizations/meds for your trip will be pre-checked on the left side of the Immunization Form. If you are current, fill in the date you were immunized in the “current” column. A list of immunizations and their effective immunity is on the reverse side of this paper.
STEP 3: Check to see if you can get immunizations through your own health insurance, often this avenue is more affordable. Call your insurance now to begin scheduling an appointment. Appointments often need to be scheduled weeks or months in advance to allow time for your doctor to order the immunizations. If you are certain you will acquire any/all immunizations through your own medical provider, initial in the “Will acquire outside PLNU box”. You will not able to order that immunization through PLNU at a later point in the semester if you check this box.
STEP 4: If you would like to order immunizations through PLNU LoveWorks, mark the Wellness Center Order or Shelter Island Order column (depending on the med) and write in the corresponding price on the far right column. A list of our immunization prices is listed on the reverse side. PLNU offers immunizations at cost and uses Shelter Island Medical Group for immunizations that we are not legally able to administer.
