Student Employment Authorization Form
This is the first form you should receive from your supervisor once you have been hired is the Student Employment Authorization Form. (These forms are available to all supervisors through the Student Employment Office.) This is a quadruplicate form that has three major sections: A Job Responsibility Statement, a section to be completed by the student and a section to be completed by the supervisor. This form should be completed at the beginning of each new school year, at the beginning of the summer (for those who will stay on campus and work during the summer), and whenever a student starts a new job on campus.
When the form is completed, it should be brought to the Student Employment Office, located in the OSV on the third floor of the Ryan Learning Center in Suite 306. The Associate Director will process the form in conjunction with the other two required forms.

This is an IRS form that must be completed before a student can officially be added to the PLNU payroll. The main question that this form asks is how many allowances the student wants to claim for tax purposes. This form needs to be completed the first time a student is hired on campus and will remain on file for the duration of this student’s time at the university. A new one only needs to be done if something has changed (i.e. the student got married.) This form will be completed in the Student Employment Office and then sent to payroll with the yellow copy of the Student Employment Authorization Form.

This form is required by the U.S. Department of Justice. It is used to establish both the student’s identity and his/her eligibility to work in the United States. Certain documentation is required to prove identity and eligibility. Most frequently used documents are a passport, OR a driver’s license AND Social Security Card. These must be original documents, no photocopies or faxes. Click here for a complete list of acceptable documents. This form must be completed before a student may begin their first job on campus and will be kept on file for the duration of the student’s time at PLNU.
