Change of Schedule

For students who need to add or drop courses after Web Registration has closed. 
Instructor's signature is required to add a course. Please return this form to Records Office.


Change of Program
For undergraduate students who need to change or update their intended degree, major, concentration, minor or catalog year.  Please submit this form to the Records Office.  If needed, new advisors are automatically assigned by the Academic Advising Office.

Change of Program (Post-Baccalaureate)
For post-baccalaureate students who need to change or update their degree program.

Diploma Mailing Request

For students who have graduated and need to request that their official diploma be mailed. 
(For replacement diplomas, please contact the Alumni Office.) 

 
Grad Check Application

Juniors have the option of completing one preliminary grad check. A final grad check is required the semester before an undergraduate student plans to graduate.

Please stop by the Records Office to meet with an Academic Records Specialist.

 
Student Verification

Official student verifications are provided to a third-party for students who need proof of enrollment or GPA verification.


Transcript Requests

For students who need official or unofficial transcripts.


Withdraw from PLNU

Required for students who will not be attending PLNU for at least one semester or who do not plan to return.
