CDC Schedule of Recommended Immunizations for Adults
Exemption to Vaccination Policy:
Students who have a medical condition that precludes them from safely complying with recommended immunization schedules should complete the PLNU provided vaccine exemption form (found in patient portal). Medical exemptions to immunization require a signature and office stamp by a licensed medical provider (MD, DO, NP, PA) as well as a written statement in order to document that the physical condition of the student is such, or medical circumstances relating to the student are such, that immunization is not considered safe.
Students forgoing vaccination without medical cause, must submit the provided exemption form without a provider signature/office stamp indicating that the student is requesting exemption to vaccination for reasons other than a medical contraindication which may include religious, moral, or personal reasons.
Immunization Exemption Request Form
Please note:
- Medical exemption forms must include a written statement by a licensed medical provider documenting the immunization status
- If the student is residential and under 18: Exemption requests require the document signed by both the student and the parent or guardian
Steps For Submitting Documentation
Simply pick the platform that is easiest for you
Option A: The Volta Health Portal
Point Loma Nazarene University is excited to partner with Volta Health to streamline our immunization compliance process. This new, faster system allows you to easily submit your records and clear your registration holds. To get started, all you need is your current health insurance information.

- Step 1: Access – Download the Volta app (iPhone/Android) or go to student.volta.health.
- Step 2: Submit Records – Go to Immunization Compliance > Immunizations > Select the requirement, and choose one of these:
- Manual: Upload a photo/file of your records.
- Auto-Import: Log into your doctor's portal through the app to sync your history automatically.
- Step 3: Track – Wait 3–5 business days for review and check your status in the app.
- Support: If you have questions regarding account access, uploading records, compliance status, or technical issues, contact the Volta Support Team at support@volta.health.

The California Immunization Registry (CAIR) website acts as a centralized, secure, statewide system that helps healthcare providers, schools, and families manage and access immunization records. It ensures that vaccination histories are maintained even if a person switches doctors, moves, or loses their paper "yellow card".
*If you or your child was vaccinated in another state, CAIRS will NOT have your records digitally. All medical facilities in CA are required to upload all immunizations given to the CAIRS database.
Here is how the CAIR website helps with immunization records:
For Patients and Parents
- Digital Vaccine Record (DVR) Portal: Users can access their own or their child’s immunization record, including COVID-19 vaccines, directly through a web browser.
- Convenience: It allows for quick retrieval of records for school, camp, or travel, without needing to call a doctor’s office.
- Record Integrity: It ensures that records from different providers across the state are consolidated into one place.

As a consumer of student health services, you have responsibilities as well as rights. They include the following:
- We are a confidential resource as detailed by law; Health Insurance Portability and Accountability Act (HIPAA).
- We do not bill insurance. Proof of health insurance is a University requirement for emergency purposes and in order for Wellness staff to assist students in receiving prescriptive medication and to facilitate medical referrals. Prescriptive medications, treatments, and laboratory tests will have a financial charge. Charges will be billed to your student account as ‘Wellness Center Charge’. The cost is kept as low as possible. It is your responsibility to discuss any financial burden this may pose. It is your responsibility to consult your insurance plan and policy holder for cost comparisons.
- Being a good health consumer does not mean being a silent partner. If you feel your rights have not been respected, ask to see the Director of Wellness or the Dean of Counseling & Student Care.
- Be honest and direct in a courteous fashion about everything that relates to your health needs so that we can give you the best possible care.
- Be sure you understand your health situation. If you do not understand the treatment plan or test, ask your Wellness provider about it. Follow the prescribed plan. It is your responsibility to inform us if you cannot follow it.
